We have been listening to our patient’s feedback and amended our appointment request forms to allow patients wishing to seek a routine appointment access during core opening hours on the routine get help for any health problem form.

For patients wishing to access a same day appointment, there is a separate form to use.

Both forms are clearly identifiable and support better patient access during our opening hours.
